Eligibility:

• Voluntary, home visiting program for pregnant women or families with a newborn less than two weeks of age
• Family is asked to commit to weekly home visits for the first six months
• Family can remain in the program up to five years
• Family must live in the following zips codes:
  • 33602, 03, 04, 05, 07, 10, 12, 13, 14, 15, 17, 19, 24, 34 & 37
  • 33503, 10, 11, 34, 69, 70, 78, 79, 84 and 98

Referral: Complete the Healthy Families Community Referral from Child Welfare System (CPS) for eligibility interview.

Limitations: Do not refer if family has:

• Active, open HKI involvement.
• An open investigation that will likely result in an out-of-home placement of the targeted infant.
