Prenatal Risk Screen: A 4-part NCR form that must be completed by the health care provider for every pregnant woman in the state of Florida, pursuant to Florida Statute 383.12(1) and 383.011(1)(e). Return Envelopes for Prenatal Risk Screen: #10 envelopes with a preprinted address for providers to return their Prenatal Risk Screens via mail to the Health Department.
